Steps

1. See your doctor Complete a preventative or wellness exam with a physician between January 1 and October 31, 2026. A routine annual physical counts.

2. Get it verified Bring the PCP Form with you to your appointment and ask your doctor’s office to complete it. The form is built to capture exactly what we need — nothing more.

3. Send it in Submit your completed PCP Form by October 31, 2026. Upload it below, or email it to health@snellventures.com.
